Little Cloud Playhouse – Participant Waiver and Release of Liability *

1. Acknowledgment of Risk I understand that participation in soft play activities involves inherent risks, including but not limited to: falls, collisions, bumps, trips, slips, or other injuries. I acknowledge that Little Cloud Playhouse, LLC has taken reasonable steps to provide a safe environment but cannot eliminate all risks associated with play. 2. Assumption of Responsibility I agree that:     •    Parents/guardians are responsible for supervising their child(ren) at all times.     •    Socks are required in all play areas.     •    No roughhousing, inappropriate behavior, or use of equipment in unintended ways is permitted.     •    Little Cloud Playhouse staff may remove participants who do not follow safety rules. 3. Medical Authorization In the event of an injury, I authorize Little Cloud Playhouse staff to administer basic first aid and/or seek emergency medical treatment if necessary. I understand that I am fully responsible for any medical costs incurred. 4. Release of Liability By signing this waiver, I release and discharge Little Cloud Playhouse, LLC, its owners, employees, and affiliates from any and all liability, claims, demands, or causes of action arising from participation in activities at the facility, except in cases of gross negligence or willful misconduct. 5. Agreement I have read and understood this waiver. I voluntarily accept the terms and conditions for myself and my child(ren).
